ID card and Medical Aid Card - Personal Details will be asked to open a medical file, contact number, email and physical adress. Chronic list of medication you are currently using, or clinic book and relevant medical history information you may have from previous doctors.

Please arrive 15min prior to appointment(This is to allow time to update current patient file / open new patient file with reception)

We are contracted with most medical aids, please follow up with your medical aid in requesting Designated Service provider (Dr AM Schoeman practice 1548336)

Should you run late for your appointment or can not make your appointment please call our reception and make us aware of the situation.

If you have a medical emergency (Life and Death Emergency) please phone the practice on 042 283 0262 and state that you are on your way with an said Emergency - Please come right away.

Refilled Prescriptions, please note that all Chronic Prescriptions needs to be renewed every 6month period - This will only be done via your 6month follow-up consultation with doctor. No Prescription will be renewed without a Doctor's consultation.
